Overview: PEP (Post-exposure Prophylaxis) is an emergency antiretroviral treatment taken after potential exposure to HIV (human immunodeficiency virus). It is designed to prevent HIV infection in individuals who may have been exposed to the virus through unprotected sex, needle-sharing, or occupational exposure, such as a healthcare worker’s needlestick injury. PEP is most effective when started within 72 hours of exposure.

Active Ingredients: PEP typically consists of a combination of antiretroviral medications from different classes. A common regimen includes:

  • Truvada (Tenofovir disoproxil fumarate 300 mg / Emtricitabine 200 mg)
  • Tivicay (Dolutegravir 50 mg)

Formulation: PEP comes in the form of oral tablets. The medication regimen usually involves taking two or three drugs daily for 28 days.

Mechanism of Action: PEP works by preventing HIV from replicating and establishing an infection after potential exposure. The combination of drugs targets different stages of the virus's lifecycle, reducing the chance that it can spread within the body.

Indications: PEP is recommended for individuals who have potentially been exposed to HIV within the last 72 hours, including:

  • Unprotected sexual contact with an HIV-positive individual or someone with unknown HIV status
  • Condom breakage or slippage during sex
  • Sharing needles or other drug injection equipment
  • Occupational exposure, such as needlestick injuries in healthcare settings

Usage:

  • PEP must be started as soon as possible, ideally within 2 hours, but no later than 72 hours after potential HIV exposure.
  • It is taken for 28 consecutive days, and it’s crucial to complete the full course for maximum effectiveness.
  • PEP is not a substitute for regular HIV prevention methods, such as PrEP, condoms, or safe needle practices.
